IN THE NAME AND BY THE AUTHORITY OF THE CITY OF MONTICELLO, IOWA
RESOLUTION #18-02
Approving Pay Request #4 in the amount of $1,947.50 and Change Order #3 in the amount of $2,050.00, submitted by Eastern Iowa Excavating & Concrete Re: 2017 Main Street HMA Project.
WHEREAS, Eastern Iowa Excavating & Concrete contracted with the City to perform the 2017 Main Street HMA project, and
WHEREAS, Eastern Iowa Excavating & Concrete has submitted their 4th pay request in the amount of $1,947.50 which reflects 5% retainage, with total retainage held to date after this payment in the amount of $27,762.63, and
WHEREAS, Eastern Iowa Excavating & Concrete has also submitted Change Order #3 in the amount of $2,050.00, said Change Order being tied to contract language that provides an incentive, or additional payment, to the contractor if the contractor meets certain pavement smoothness standards, and
WHEREAS, The City will be reimbursed a large portion of this expense after the submission of a request for reimbursement through the IDOT, and
WHEREAS, The City Engineer has reviewed Pay Request #4 and Change Order #3 and recommends their approval.
NOW THEREFORE, BE IT RESOLVED that the City Council of Monticello, Iowa does hereby approve Pay Request #4 submitted by Eastern Iowa Excavating & Concrete, Re: 2017 Main Street HMA project, and authorizes payment in the amount of $1,947.50, maintaining a 5% retainage that totals $27,762.63 after this payment, and also approves Change Order #3 in the amount of $2,050.00.
